Question to doctors in yahoo answers on primary immunodeficiency?
What is the line of management?
1 yr 7 months boy with 2 episode of left lateral neck abcess.
Unhealing wound over the I&D site.
Onycholychosis on finger and toe nails.
Oral thrush. Also comes with intermittent fever, intermittent URTI, chronic diarrhoe. Painful Erythematous rash scalding looking over the flexor surface of neck, axillae, and groin.All these symptoms started when the child was 6months old. Height and weight at 25th centile of growth chart. Vitals are normal. PICCOL negative. Rest of the systemic examination is normal except for the skin findings mentioned above. FBC- lymphocytosis. LFT shows hypoalbunemia, ALP 540U/L. RFT low sodium. Immunoglobulin level normal. Retrovirus negative. High TSH. Currently under antifungal treatment. Or could it just be a skin infection, vitamin defienciency as diagnosis?

High TSH signifies LowT3/T4.Genes responsible for the expression of thyroxine,may be in close proximity to the cellular immunity expression (perhaps naive T cells)genes may get altered/deleted resulting something like contiguous deletion syn..
Again,low T3 could mean less green signal for the expression of immune globulins et al,just as the ‘permissive role’ seen in many body processes(Beta receptor surfacing etc).
