Does anyone have an health release form for babysitter?
We are going on vacation and leaving both grandma’s in charge of our children. I want to leave them a “in case anything happens” they can make medical desicions about them. Does anyone have a medical release form/ emergency form? I want to give them the okay for only when we are on vacation also with the insurance, doctors, medicine information on it.
You can probably even just print out a paper that you make yourself in Microsoft Word or Corel or whatever program you have saying that you are leaving your children in the care of your parents. Then say that you authorize them to seek medical attention when necessary and provide them with the information. Also, if you can leave an insurance card with them (if you have an extra or just photocopy it).
